All new patients need to fill out a Registration Form and a Medical History Form. These can be obtained from reception and you will need to fill in a separate form for each new patient who wishes to register. To save you time, you can download the forms here and print them off to be filled in at home. You can then bring the completed forms with you when you come in to register.

If you wish to register with us, we’ll ask you to fill out a Registration Form and a Medical History form. You can obtain these from our friendly reception team – or simply download them here and bring them along to your first appointment.
What to expect at Eurodental
Clarity
During your new patient consultation, we’ll check your teeth, gums and mouth, assess your gum health, screen for oral cancer and, if necessary, take x-rays. We will also take a detailed medical and dental history. After listening to your concerns and discussing treatment options, we will create a personalised treatment plan for you. At your next visit, we will clarify any questions you may have about the treatment plan and its outcomes before starting your treatment.
Care for the whole family
As a family practice, we take great pleasure in treating multi-generation families and especially welcome children. We go out of our way to make children’s dentistry enjoyable, fun and pain-free – and don’t forget, we can see them for free on the NHS until they turn 18. We believe in encouraging good habits early on and take a preventive approach to treatment. We like to see children as young as possible to encourage good brushing and healthy eating.
Comprehensive NHS care
As an NHS patient, you can be confident that we will not only aim to provide you with excellent NHS dentistry, but that you’ll also benefit from great customer care in a relaxed environment. NHS dental care is designed for everyone, whatever your age or however long it has been since you last saw a dentist. NHS dentists are there to provide the treatment that is necessary to maintain the health of your mouth, teeth and gums.
Help for exempt patients
If you do not need to pay NHS charges, you will need to bring your exempt documentation to your first appointment. The current exemptions (including for children) will not change under the new arrangements. Our practice staff will be able to assist with any enquires you have regarding payment exemptions.
